ABSTRACT

Objective: To evaluate the therapeutic efficacy of hematopoietic stem cell transplantation (HSCT) for Wiskott-Aldrich syndrome (WAS), and to analyze the factors related to the outcomes. Methods: The clinical data of 60 children with WAS received HSCT in Shanghai Children's Medical Center from January 2006 to December 2020 were retrospectively analyzed. All cases were treated with a myeloablative conditioning regimen with busulfan and cyclophosphamide, and a graft-versus-host disease (GVHD) prevention regimen based on cyclosporine and methotrexate. Implantation, GVHD, transplant-related complications, immune reconstitution and survival rate were observed. Survival analysis was performed by Kaplan-Meier method, and Log-Rank method was used for univariate comparison. Results: The 60 male patients had main clinical features as infection and bleeding. The age at diagnosis was 0.4 (0.3, 0.8) years, and the age at transplantation was 1.1 (0.6, 2.1) years. There were 20 cases of human leukocyte antigen matched transplantation and 40 mismatched transplantation; 35 patients received peripheral blood HSCT, and 25 cord blood HSCT. All cases were fully implanted. The incidence of acute GVHD (aGVHD) was 48% (29/60) and only 2 (7%) developed aGVHD of grade Ⅲ; the incidence of chronic GVHD (cGVHD) was 23% (13/56), and all cases were limited. The incidence of CMV and EBV infection was 35% (21/60) and 33% (20/60) respectively; and 7 patients developed CMV retinitis. The incidence of sinus obstruction syndrome was 8% (5/60), of whom 2 patients died. There were 7 cases (12%) of autoimmune hemocytopenia after transplantation. Natural killer cells were the earliest to recover after transplantation, and B cells and CD4+T cells returned to normal at about 180 days post HSCT. The 5-year overall survival rate (OS) of this group was 93% (95%CI 86%-99%), and the event free survial rate (EFS) was 87% (95%CI 78%-95%). EFS of non-CMV reactivation group is higher than that of CMV reactivation group (95% (37/39) vs.71% (15/21), χ2=5.22, P=0.022). Conclusions: The therapeutic efficacy of HSCT for WAS is satisfying, and the early application of HSCT in typical cases can achieve better outcome. CMV infection is the main factor affecting disease-free survival rate, which can be improved by strengthening the management of complications.

ABSTRACT

Bisphenol A is a common environmental factor and endocrine disruptor that exerts a negative impact on male reproductive ability. By exploring bisphenol A-induced testicular cell death using the Institute of Cancer Research (ICR) mouse model, we found that a ferroptosis phenomenon may exist. Mice were divided into six groups and administered different doses of bisphenol A via intragastric gavage once daily for 45 consecutive days. Serum was then collected to determine the levels of superoxide dismutase and malondialdehyde. Epididymal sperm was also collected for semen analysis, and testicular tissue was collected for ferritin content determination, electron microscope observation of mitochondrial morphology, immunohistochemistry, real-time quantitative polymerase chain reaction, and western blot analysis. Exposure to bisphenol A was found to decrease sperm quality and cause oxidative damage, iron accumulation, and mitochondrial damage in the testes of mice. In addition, bisphenol A was confirmed to affect the expression of the ferroptosis-related genes, glutathione peroxidase 4 (GPX4), ferritin heavy chain 1 (FTH1), cyclooxygenase 2 (COX2), and acyl-CoA synthetase 4 (ACSL4) in mouse testicular tissues. Accordingly, we speculate that bisphenol A induces oxidative stress, which leads to the ferroptosis of testicular cells. Overall, the inhibition of ferroptosis may be a potential strategy to reduce male reproductive toxicity caused by bisphenol A.

ABSTRACT

Chronic stress leads to many psychiatric disorders, including social and anxiety disorders that are associated with over-activation of neurons in the basolateral amygdala (BLA). However, not all individuals develop psychiatric diseases, many showing considerable resilience against stress exposure. Whether BLA neuronal activity is involved in regulating an individual's vulnerability to stress remains elusive. In this study, using a mouse model of chronic social defeat stress (CSDS), we divided the mice into susceptible and resilient subgroups based on their social interaction behavior. Using in vivo fiber photometry and in vitro patch-clamp recording, we showed that CSDS persistently (after 20 days of recovery from stress) increased BLA neuronal activity in all the mice regardless of their susceptible or resilient nature, although impaired social interaction behavior was only observed in susceptible mice. Increased anxiety-like behavior, on the other hand, was evident in both groups. Notably, the CSDS-induced increase of BLA neuronal activity correlated well with the heightened anxiety-like but not the social avoidance behavior in mice. These findings provide new insight to our understanding of the role of neuronal activity in the amygdala in mediating stress-related psychiatric disorders.

ABSTRACT

Objective:To explore the experience of home dietary changes in postoperative patients with gastric cancer.Methods:From December 2019 to February 2020, purpose sampling method was used to select 13 postoperative gastric cancer patients from the Oncology Department of Taixing People's Hospital in Jiangsu Province to conduct semi-structured interviews. The Colaizzi seven-step analysis method was used to analyze the interview data and extract topics.Results:Three main themes were summarized: reconstruction of dietary habits, lack of pleasure in eating, and continuous dietary nursing needs.Conclusions:There are many dietary problems in postoperative patients with gastric cancer. Medical staff should pay attention to dietary changes of patients and take targeted intervention measures, in order to increase the positive experience of dietary.

ABSTRACT

Objective:To study the effects of Fangji Huangqitang(FJHQT) on migration, adhesion,invasion and tube formation of human umbilical vein endothelial cells (HUVECs) induced by vascular endothelial growth factor (VEGF). Method:HUVECs were induced by VEGF (20 μg·L<sup>-1</sup>) <italic>in vitro</italic>. The effects of FJHQT (0.25,0.5,1 g·L<sup>-1</sup>) on HUVECs were detected by methyl thiazolyl tetrazolium(MTT), scratch repair, transwell migration, adhesion, invasion and tube formation. Protein in HUVECs was extracted and protein expression levels of phosphorylated Janus kinase 1 (p-JAK1) were detected by Western blot. Result:Compared with control group, VEGF (20 μg·L<sup>-1</sup>) can increase the proliferation, scratch repair, transwell migration, adhesion, invasion and tube formation of HUVECs cells (<italic>P</italic><0.01), compared with VEGF group, FJHQT (0.25,0.5,1 g·L<sup>-1</sup>) ,there is no significant effect on the proliferation of HUVECs induced by VEGF for 24 hours, but it can significantly reduce the scratch repair, migration, adhesion, invasion and tube formation of HUVECs induced by VEGF within 24 hours (<italic>P</italic><0.05). Compared with blank group, VEGF could induce abnormal elevation of p-JAK1 in HUVECs (<italic>P</italic><0.01), while FJHQT (0.25,0.5,1 g·L<sup>-1</sup>) could significantly reduce the expression levels of p-JAK1 (<italic>P</italic><0.05,<italic>P</italic><0.01). Conclusion:FJHQT can inhibit the migration, adhesion and invasion of HUVECs, the mechanism may be related to JAK1.

ABSTRACT

Objective@#To investigate the efficacy of alternative donor (AD) in the treatment of aplastic anemia (AA) in children.@*Methods@#The clinical data of AA children who received AD HSCT in our center from Apr. 2010 to Dec. 2016 were retrospectively analyzed. The overall survival (OS) rate, implant success rate, incidence of acute and chronic graft-versus-host disease (GVHD) were statistically analyzed.@*Results@#A total of 109 children with acquired AA, including 64 severe AA (SAA) , 32 very severe AA (VSAA) and 13 transfusion dependent non-severe AA (NSAA) , were recruited in this retrospective AD HSCT study, the median age was 6 (0.8-18) years old. Of them, 44 patients with 10/10 matched unrelated donor (MUD) , 44 patients with mismatched unrelated donor (MMUD) and 21 patients with mismatched related donor (MMRD) . All patients did not receive ATG before HSCT and the active infection was excluded. Except 3 patients suffered from a second graft failure (2 of them rescued by second HSCT) , 106/109 (97.2%) were engrafted with neutrophil and platelet recovery occurring at a median of 13 days (range, 9-19) and 16 days (range, 10-81) post-transplant. Until day 100 post transplantation, the incidence was 74.3% (81/109) for acute GVHD (aGVHD) and 39.4% (43/109) for grade Ⅱ-Ⅳ aGVHD, 30.7% (31/101) and 9.9% (10/101) for overall chronic GVHD (cGVHD) and moderate cGVHD, respectively, and nobody developed an extend cGVHD. After median follow up of 39 (0.7-103) months for all patients, 13 of 109 patients died. The estimated 5-year overall survival (OS) of the entire cohort was 88.1% (95%CI 81.1%-91.4%) with no difference among the MUD, MMUD and MMRD cohort (93.2%, 84.1% and 85.7%, respectively, P=0.361) .@*Conclusion@#These excellent outcomes suggest that unmanipulated AD PBSC is a good HSCT source for children with SAA. It’s reasonable to consider AD HSCT as first line therapy for SAA children without matched sibling donor. Better strategies are required to prevent GVHD.

Objective:To explore the effect of Ermiaosan(EMS) on the polarization of M1 by lipopolysaccharide(LPS)+interferon(IFN)-γ and M2 induced by interleukin(IL)-4+IL-13 in rat bone marrow-derived macrophages. Method:Macrophages from rat bone marrow were extracted in vitro, stimulated by macrophage colony stimulating factor(M-CSF), induced to macrophages (marked by F4/80), stimulated by LPS+IFN-γ and induced to polarize to M1,while stimulated by IL-4+IL-13 and induced to polarize to M2. After adding different concentrations of EMS (0.2,0.4,0.8 g·L-1), the phenotypes of M1 and M2 were detected by immunofluorescence, and the effect of EMS on M1(marked by CD68 and iNOS)/M2(marked by CD206 and Arginase) polarization of macrophages from rat bone marrow was detected. Result:Compared with control group, LPS + IFN-γ could increase the polarization of M1 (P<0.01),while IL-4+IL-13 could increase the polarization of M2 (P<0.01); compared with LPS+IFN-γ/IL-4+IL-13 group, EMS (0.2,0.4,0.8 g·L-1) could inhibit the polarization of M1 induced by LPS+IFN -γ for 24 hours (P<0.05), but had no significant effect on polarization of M2 induced by IL-4+IL-13. Conclusion:EMS can inhibit M1 polarization induced by LPS+IFN - γ, but has no effect on M2 polarization induced by IL-4+IL-13.

ABSTRACT

Objective::To study the effects of Ermiaosan on migration, adhesion and invasion of human fibroblast-liked synovial cells(FLS) and explore its mechanism. Method::Using the human FLS as the research object, the nontoxic concentration of FLS.FLS was determined by methyl thiazolyl tetrazolium (MTT) colorimetric assay for the follow-up experiment. The transwell migration, adhesion and transwell invasion test were used to detect the migration and adhesion of the different concentration of Ermiaosan on FLS, respectively. The expression of interleukin (IL)-1 beta of FLS supernatant was detected by enzyme linked immunosorbent assay (ELISA). Protein in FLS was extracted and protein expression levels of phosphorylated Janus kinase 1 (p-JAK1), p-signal transducer and activator of transcription (STAT1) and p-STAT6 were detected by Western blot. Result::Compared with control group, tumor necrosis factor(TNF)-α (20 μg·L-1) increased the proliferation, migration, adhesion, invasion and the secretion of IL-1β of FLS (P<0.01). Ermiaosan(0.2, 0.4, 0.8 mg·L-1) had no significant effect on the proliferation of FLS induced by TNF-α for 24 h. Within 24 h, the migration, adhesion, invasion, invasion, and secretion of IL-1β of FLS cells induced by TNF-α were also decreased significantly(P<0.05, P<0.01). Compared with the blank group, TNF-α could induce abnormal elevation of p-JAK1, p-STAT1 and p-STAT6 in FLS (P<0.01), while Ermiaosan of 0.2, 0.4, 0.8 g·L-1 could significantly reduce the expression levels of p-JAK1, p-STAT1 and p-STAT6 (P<0.05, P<0.01). Conclusion::Ermiaosan can inhibit the migration, adhesion and invasion of FLS, and its mechanism may be related to the inhibition of the secretion of IL-1β, the mechanism may be related to JAK/STAT pathway.

ABSTRACT

OBJECTIVE: To study the effects of curcumin on gemcitabine (GEM)-resistant pancreatic cancer SW1990 cells and its mechanism. METHODS: CCK8 assay was used to detect the effects of different concentrations of GEM (50, 100, 150, 200, 250 μmol/L) on the survival rate of SW1990 cells and GEM-resistant SW1990 cells (SW1991/GEM resistant cells); half inhibitory concentration (IC50) and drug resistance multiple were calculated. CCK8 assay was performed to detect the effects of different concentrations of curcumin (1, 5, 10, 20, 40 μmol/L) on survival rate of SW1990/GEM resistant cells, and IC50 was calculated. CCK8 assay was used to detect the effects of curcumin 2.41 μmol/L combined with different concentrations of GEM (25, 50, 75, 100, 125 μmol/L) on the survival rate of SW1990 cells and SW1990/GEM resistant cells, and IC50 and drug resistance reversal fold of GEM were calculated. Flow cytometry was carried out to detect the cell cycle distribution and apoptosis rate of SW1990 after treated with GEM alone or curcumin (2.41 μmol/L) combined with GEM using IC50 of GEM as drug concentration. Western blot assay was used to the protein expression of FAS, AKT, p-AKT, PI3K, p-PI3K, Caspase-3, Bcl-2 and related X protein (Bax). RT-PCR was used to detect mRNA expression. RESULTS: IC50 of GEM to SW1990 cells was 92 μmol/L. IC50 of GEM to SW1990/GEM resistant cells was 216 μmol/L, and drug resistance multiple SW1990 cells to GEM was 2.35. IC50 of curcumin to SW1990/GEM resistant cells was 9.2 μmol/L. Under 2.41 μmol/L curcumin, IC50 of GEM to SW1990 cells was 75  μmol/L, and IC50 of GEM to SW1990/GEM resistant cells was 98 μmol/L; drug resistance reversal multiple of SW1990/GEM resistant cells to GEM was 2.2. Compared with GEM alone, the apoptosis rate of SW1990 cells and SW1990/GEM resistant cells were increased significantly after curcumin combined with GEM (P<0.05), blocking at G0/G1 phase; the protein expression of FAS, p-AKT, p-PI3K and Bcl-2 and mRNA expression of FAS and Bcl-2 were decreased significantly  (P<0.05); the protein and mRNA expression of Bax      and Caspase-3 were increased significantly (P<0.05). CONCLUSIONS: Curcumin can reverse drug resistance of SW1990 cells to GEM, the mechanism of which may be associated with PI3K/AKT pathway.

ABSTRACT

Objective To explore the influence of social network on the quality of life of elderly stroke patients in communities. Methods The general situation questionnaire, the stroke impact scale ( SIS) and social network scale for the elderly were used to investigate the community elderly stroke patients. Results The total score of social network was(39. 82±5. 87). The scores of social participation,social con-tact and social support were(11. 79±1. 61),(14. 90±3. 31) and (13. 13±3. 19). Results of univariate a-nalysis showed there were statistically significant differences ( P<0. 05) in quality of life among stroke pa-tients of different ages (F=7. 803),educational level (F=2. 664),family income (F=4. 754),smoking (F=5. 467),alcohol consumption (F=7. 058),physical exercise (F=10. 393),body mass index (t=4. 841) and hypertension (t=-2. 862). Multiple linear regression results showed that 3 times or more exercises per week ( B=5. 689),normal body mass index (B=6. 613),high degree of social support (B=0. 710),social partici-pation (B=1. 421) and social contact (B=1. 372) were protective factors for the quality of life of stroke pa-tients. Age 80 years and above (B=-10. 426),frequent smoking (B=-9. 005),frequent drinking (B=-8. 419) and hypertension ( B=-5. 224) were risk factors for quality of life. Conclusion The elderly stroke patients should quit smoking and alcohol,strengthen exercise,maintain a normal body mass index and avoid overweight and obesity in daily life. While promoting health-related behaviors,more attention should be paid to the promotion effect of social network on treatment,rehabilitation and overall quality of life of stroke patients.

ABSTRACT

Objective To analyze the influencing factors of venous thromboembolism (VTE) in intensive care units, and provide evidence for corresponding clinical decisions. Methods A case-control study was carried out:224 VTE patients admitted to the ICUs of tertiary hospitals in Kunshan from Nov. 2011 to Nov. 2016 were included in the case group, and 224 non-VTE patients admitted during the same period were randomly selected as the control. The patients' medical history, laboratory tests, prophylaxis and treatment of VTE, and other relevant data were retrieved. Logistic aggression analysis was utilized to identify the influencing factors of VTE in hospitalized critically ill patients. Results The univariate analysis showed that gender, age, hypertension, smoking, D-dimer, Caprini scaling, prophylaxis and treatment of VTE were associated with VTE. The multivariate analysis indicated that except hypertension, the other varieties were independent influencing factors of VTE in hospitalized critically ill patients:female ( OR=1.68, 95%CI:1.09-2.61, P=0.02), higher age(OR=1.03, 95%CI:1.01-1.04, P<0.01), smoking(OR=6.82, 95%CI:1.70-27.46, P=0.007), smoking(OR=6.82, 95%CI:1.70-27.46, P=0.007), D-dimer(OR=0.94, 95%CI:0.92-0.96, P<0.01), higher Caprini scaling(OR=1.80, 95%CI:1.33-2.44, P<0.01), prophylaxis and treatment of VTE(OR=0.34, 95%CI:0.15-0.79, P=0.01). Conclusions Those ICU patients who are female, elder, with smoking history, have higher test value of D-dimer should be screened and assessed for VTE, and those with higher Caprini scaling should be given closer attention, and receive corresponding prophylaxis and treatment to reduce the formation of VTE and its damage.

ABSTRACT

Objective To evaluate the application value of low-dose CT (LDCT) in physical examination for coal miners with different exposure time.Methods The consecutive three-year imaging data of 972 coal miners with over 20-year exposure were retrospectively reviewed.All miners were divided into 3 groups according to different exposure time,including 317 cases with less than 10 years,299 cases with 10-20 years and 356 cases with more than 20 years.All subjects underwent LDCT examination every year during three consecutive years.Results Baseline LDCT:As the exposure time was prolonged (with less than 10 years,10-20 years and more than 20 years),the number of coal miners has increased with non-calcified pulmonary nodules,interstitial pulmonary lesions,clinically cured or stable stage of pulmonary tuberculosis,pulmonary calcification and pulmonary fibrous stripes,but without statistically significant difference.The detection rates of lung bullae,aorta and coronary artery sclerosis were also increased mildly,with the prolonged exposure time.There was no significant correlation between exposure time and detection rates of bronchiectasis,pulmonary inflammatory lesions,lymph node enlargement or calcification,pleural lesions.The exposure time of 6 cases of malignant nodules and 2 cases of active tuberculosis was more than 10 years.During a two-year follow-up using LDCT scan,there were no significant changes in most of intrapulmonary,pleural and mediastinal lesions.Neither were in pulmonary nodules less than or equal to 4 mm.Three pulmonary nodules with 4-8 mm diameters were enlarged.Most of the inflammatory lesions have changed in size.Conclusion The LDCT scan has a certain value for chest physical examination of the coal miners with different exposure time.It is very necessary to screen the high-risk population of coal miners with exposure time of more than 10 years using LDCT.LDCT reexamination has significant value for pulmonary nodules and inflammatory lesions with diameter of more than 4 mm.Most of intrapulmonary,pleural and mediastinal lesions have no obvious change in the short term,and make an annual reexamination unnecessary,neither does a negative baseline LDCT.

Objective To analyze the index of relative apparent diffusion coefficient (rADC)of magnetic resonance diffusion weighted imaging (DWI)for nasopharyngeal carcinoma with cervical metastatic lymph node before and after treatment,assess the application value of rADC in the recent curative effect after radiotherapy and chemotherapy.Methods 45 cases of nasopharyngeal carcinoma with cervical metastatic lymph nodes proved pathologically were selected as the research objects,which was including 107 pieces of metastatic lymph nodes and underwent DWI.The rADC index of cervical metastatic lymph nodes before and after radiotherapy and chemotherapy was measured and calculated.The cervical metastatic lymph nodes were divided into effective group (CR+PR,n=84)and ineffective group(SD+PD,n=23),and the difference of the rADC index between the two groups was compared.Results For the target volume disappeared or reduced more than 30% in the effective group after treatment,the rADC index was significantly higher than that before treatment(t=4.981, P<0.05).The rADC difference between effective group and ineffective group was statistically significance (t=3.471,P<0.05).Conclusion rADC can be used as the auxiliary marker for the evaluation of short-term curative effect of nasopharyngeal carcinoma with cervical metastatic lymph nodes.

To analyze the influence of external social capital on organizational performance of civil society organ-izations in HIV/AIDS prevention and control fields. Methods:Multi-variate stratified sampling and cluster sampling methods were used to investigate 212 civil society organizations in the field of AIDS prevention and control in eight prov-inces of China. The data were analyzed with the help of the multivariate logistic regression. Results:The findings showed that the shared vision,network will affect organizational institution performance(OR=3.23,95% CI,1.77-5.88),(OR=2.21,95% CI:1.21-4.02); Shared vision factors will affect the organization's performance of the organization's external social capital (OR=4.17,95% CI:2.23-7.78);Shared vision is closely relative to Shared vision and the organization's financial performance (OR=2.97,95% CI:1.60-5.51);Shared vision, network and support has positive correlation with organizational culture performance(OR=2.04,95% CI:1.09-3.85),(OR=2.02 ,95% CI:1.09 -3.76), (OR =3.34, 95% CI: 1.79 -6.23); Shared vision and the network will affect the organization's comprehensive performance (OR=14.56,95% CI:6.83-31.06), (OR=2.23,95% CI:1.11-4. 48). Conclusions:The external social capital of civil society organizations in the field of HIV/AIDS prevention and con-trol can affect organizational performance,and different organizations and the governments should develop the external social capital vigorously to improve organizational performance.

ABSTRACT

Objective To study the relationship between psychological capital and quality of life (QL) in cancer patients and search for better corresponding nursing strategies. Methods Investigating psychological capital state in cancer patients, and analyzing the relationship between social support score and QL. Results Cancer patients′ positive psychological capital score were low: The total score were 74.33 ± 20.04, self-efficacy 20.87 ± 6.64, hope 22.12 ± 6.04, toughness 20.35 ± 5.65, and optimism 12.36 ± 5.55. Cancer patients had low quality of life, quality of life scores were 59.57±0.57, body condition 15.32± 5.90, psychological condition 47.74 ± 10.22, social function 20.46 ± 5.19,family situation 18.93 ± 8.89, total score 115.48±20.19;The psychological capital and quality of life of cancer patients were positively related (r = 0.517, P < 0.01), cancer patients dimensions of psychological capital was positively correlated with the dimensions of quality of life (r =0.189-0.517, P < 0.01). Conclusions Cancer patients′ positive psychological capital and the quality of life both need to be improved; Cancer patients′ psychological capital are closely related to the quality of life and we should pay attention to improve the patients′psychological capital so as to better their quality of life during the care process.

ABSTRACT

Objective@#To investigate the relationship between core members' social capital and performance among HIV/AIDS-related community-based organizations (CBO).@*Methods@#From July to December in 2015, a total of 327 core members from 212 HIV/AIDS-related CBO in 8 provinces were recruited based on the prevalence of HIV/AIDS (e.g., Yunnan, Hunan, and Sichuan are in high epidemic level; Anhui, Hubei, Shandong, and Jilin are in middle epidemic level; and Gansu is in low epidemic level) by multistage stratified cluster sampling and convenient sampling method. A questionnaire was administered in this study, including general demographic information, core members' social capital, individual performance and organizational performance. Multivariate logistic regression model was used to analyze the relationship between core members' social capital and performance among CBO.@*Results@#Among the 327 individuals, the proportion of male was 201(61.47%). The proportion of core members from grassroots CBO was 66.97% (219/327). Core members from non-grassroots organizations were more likely to publish articles, the OR (95%CI) was 2.58 (1.30-5.14); Social network had a positive impact on the AIDS experts, the OR (95%CI) was 2.41(1.47-3.95); Core members from registered CBO were more likely to secure funding for the organization, the OR (95%CI) was 3.42 (1.65-7.10); Social network and the core members from high endemic areas were significantly correlated with the number of HIV/AIDS patients, the OR (95%CI) were 2.79 (1.27-6.14) and 1.99 (1.21-3.27).@*Conclusions@#We should use the core members' social network to establish relationship and communication with organizations and institutions, ultimately accelerating the growth of HIV/AIDS prevention and care.

ABSTRACT

Objective@#To investigate the clinical significance of low-dose CT (LDCT) in coal mine workers with relatively long working years.@*Methods@#A total of 907 coal mine workers with ≥20 working years were enrolled, among whom there were 863 male and 44 female workers with a mean age of 49.5 years. Digital radiography (DR) was performed for these workers in 2013, and LDCT was performed for three consecutive years from 2014 to 2016.@*Results@#A total of 32 workers were found to have lung nodules by DR in 2013, while in 2014, 269 workers were found to have non-calcified lung nodules by LDCT, and there was a significant difference in the number of workers with lung nodules (χ2=233.73, P<0.005) . There was also a significant difference in the detection rate of nodules between the workers with different working years of dust exposure (χ2=6.648, P=0.00) . The male workers had a significantly higher detection rate of nodules than the female workers (χ2=5.690, P=0.017) . There was no significant difference in the number of nodules between workers with different types of work (χ2=16.985, P=0.05) . There were 443 lung nodules in total, among which 71.56% were solid nodules and 55.75% had a size of ≤4mm; malignant nodules were confirmed by surgery in 6 (0.66%) of the 907 workers after baseline LDCT. LDCT reexamination in 2015 and 2016 found new nodules in 8 workers and enlarged nodules in 3 workers, and there was no significant change in the number of nodules with a size of ≤4 mm.@*Conclusions@#It is necessary to perform high-risk population screening for coal mine workers by LDCT. The follow-up strategies for nodules with a size of ≤4mm are the same as those for negative results; annual reexamination is recommended for nodules with a size of >4-8 mm, and clinical treatment should be considered for nodules with a size of >8 mm.

ABSTRACT

Objective To evaluate the effect of different subclavian vein catheterization methods on catheter-related bloodstream infection(CRBSI) in critically ill patients.Methods Patients with subclavian vein catheterization for more than 7 days in the intensive care unit of a hospital between May 2008 and December 2015 were investigated retrospectively.They were divided into three groups:ultrasound-guided catheterization group(group A),conventional single lumen subclavian vein catheterization without skin expansion group(group B),conventional double lumen subclavian vein catheterization group(control group).The survey included name,age,diagnosis,APACHE Ⅱ score,catheterization sites and methods,whether or not succeeded in single catheterization,duration of catheterization,occurrence of CRBSI,and isolation of pathogens.Incidence of CRBSI,CRBSI per 1 000 catheter-days,and distribution of pathogens causing CRBSI were compared respectively among patients with different catheterization methods.Results A total of 2 366 patients were surveyed (group A,n =789;group B,n =786;control group,n =791).In group A,B,and control group,13,15,and 40 cases developed CRBSI respectively,incidence of CRBSI were 1.65%,1.91%,and 5.06% respectively,incidence of CRBSI per 1 000 catheter-days were 1.09‰,1.27‰,and 3.36%‰ respectively,the percentage of success in single catheterization were 97.47%,88.80%,87.23% respectively.There were significant difference in incidence,incidence of CRBSI per 1 000 catheter-days,and percentage of success in single catheterization among three groups(all P<0.01).Pairwise comparison showed that percentage of success in single catheterization in group A was higher than group B and control group,difference were significant (x2 =46.25,58.50,both P<0.01);incidence of CRBSI in control group was higher than group A and B(x52 =12.82,18.35 respectively,both P<0.01);incidence of CRBSI per 1 000 catheter-days in control group was higher than group A and B(x2 =13.74,11.22 respectively,both P<0.01).13,15,40 strains of pathogens were isolated from three groups,Staphylococcus epidermidis and Staphylococcus aureus were the main pathogens in three groups,the proportion of coagulase negative staphylococcus infection in control group was higher than group A and B.Conclusion Compared with conventional catheterization methods (single lumen,double lumen),ultasound-guided subclavian vein catheterization can effectively improve the success rate of puncture.Ultrasoundguided catheterization and conventional single lumen subclavian vein catheterization without skin expansion can reduce the occurrence of CRBSI compared with double lumen subclavian vein catheterization.

ABSTRACT

Objective To observe the therapeutic effects of medicated thread moxibustion of traditional Zhuang nationality medicine combined with Shugan Jianpi Decoction for cirrhosis ascites. Methods Sixty patients with cirrhosis ascites were randomly divided into observation group and control group by random number method, 30 cases in each group. The treatment group received medicated thread moxibustion of traditional Zhuang nationality medicine combined with Shugan Jianpi Decoction and regular Western medicine; while control group only received regular Western medicine. Two weeks was a course of treatment. The liver function (ALT, AST, TBIL, ALB, A/G), ascites integral changes and improving time of main clinical symptoms (abdominal distension, pain, anorexia, fatigue) were observed. The clinical curative effects of two groups were evaluated with 3 months of follow-up. Results The liver function (ALT, AST, TBIL, ALB, A/G) in the treatment group after treatment was significantly improved, and the ascites integral decreased, which was better than the control group, with statistical significance (P<0.05). The improving time of main clinical symptoms in the treatment group was significantly shorter than the control group (P<0.05);The clinical effective rate was 90.00%(27/30) in the treatment group and 76.67% (23/30) in the control group, with statistical significance (P<0.05). Conclusion Medicated thread moxibustion of traditional Zhuang nationality medicine combined with Shugan Jianpi Decoction has good efficacy for cirrhosis ascites.

ABSTRACT

<p><b>OBJECTIVE</b>To record the electrical activities of Antirior cingulate cortex (ACC) neurons by in vivo multi-channel recording methods using the model of complete freund's adjuvant (CFA) induced conditioned place avoidance (C-CPA), which has been set up in our previous studies.</p><p><b>METHODS</b>The electrode was self-made and the CPA responses were recorded by in vivo multi-channel recording method.</p><p><b>RESULTS</b>(1) The electrical activities of ACC neurons could be successfully recorded by the self-made electrode. (2) Before or after the injection of CFA, rats were respectively conditioned to the different place. The firing rates of ACC neurons in the CFA-paired place vs that in the non-CFA-paired place was (0.853 ± 1.377) imp/s vs (0.221 ± 0.971) imp/s (P < 0.05, n = 26). (3) The CPA responses in the CFA-paired place vs that in the non-CFA-paired place were (303.55 ± 61.77)s vs (140.32 ± 33.52)s(P < 0.05, n = 6).</p><p><b>CONCLUSION</b>The firing rates of rACC (rostral Anterior Cingulate Cortex) neurons were involved in the occurrence of the affective pain.</p>
